*Your initial premium is determined by your current age,
nearest birthday, on the December 1 which precedes or coincides with the
policy effective date. Thereafter, for purposes of determining premium,
your age will increase one year every December 1st.
**These are the current rates and reflect the current benefit structure; rates increase when you enter a new 5-year age
bracket. New York Life reserves the right to change premiums on any premium due
date or on any date when benefits change. However,
rates have only been reduced since the plan's inception in 1960.
